Food Bank Checklist

Proof of address for each adult in the home

Social Security card for each adult (age 17 and older) in the home

One of the following for minor child:
-medicaid card
-insurance card
-current school documents listing child and parent names (within 30 days)

Athens Emergency Food bank

The Ark can make a referral to the Athens Area Emergency Food Bank when the following conditions are met:

	Eligibility is twice in a six-month period
	Must be residents of Athens-Clarke County, verified by received mail with name and street, postmarked within the last thirty days. An official lease signed within the last thirty days may be used one time.
	Homeless or transients without a permanent address must obtain a referral from the Department of Family and Children Services, located on North Avenue.
	Residents of Athens Housing Authority must obtain a referral from Residential Services Office located in Rock Springs.
	Applicants with minor dependents, zero income, and no health insurance for the minors must obtain a referral from the Department of Family and Children's Services, located on North Avenue.
